What’s included

  1. Initial Contact 

Once you express interest in our services, our friendly team will reach out to you to schedule an initial meeting. We will discuss the purpose of the meeting and gather some basic information to ensure we are well-prepared to assist you effectively.


2.  Introduction and Information Exchange 

During our first meeting, either in-person or via a preferred mode of communication, we will introduce ourselves and provide an overview of our services. We'll also explain how the NDIS works and answer any questions or concerns you may have. This will be an opportunity for you to share information about your disability, your goals, and any existing supports you already have in place.


3. Comprehensive Assessment

To better understand your specific needs, our team will conduct a comprehensive assessment. This assessment will involve a series of questions and discussions to gain insight into your daily living requirements, mobility, communication preferences, health conditions, social interactions, and any other relevant aspects. We will ensure a comfortable and open environment, encouraging you to express your unique needs, desires, and aspirations.


4. Individualised Support Plan

Based on the information gathered during the assessment, our experienced team will work closely with you to develop an individualised support plan. This plan will outline the goals you want to achieve and the supports required to reach those goals. It will be tailored to your preferences, abilities, and aspirations, ensuring a person-centered approach.


5. Coordinated Support Network 

Our support coordination team will collaborate with you to establish and coordinate a network of support providers who can assist you in various areas. This may include sourcing and engaging support workers, therapists, healthcare professionals, or any other services deemed necessary to meet your unique requirements. We will facilitate communication, ensure service agreements are in place, and monitor the effectiveness of the support network.


6. Ongoing Review and Adjustments

We understand that needs and circumstances can change over time. Therefore, we will conduct regular reviews of your support plan to ensure its ongoing suitability and effectiveness. Our team will remain accessible to you, continuously monitoring your progress, addressing any concerns, and making necessary adjustments to optimise your supports.
